Automatically re-generate all profiles as part of an upgrade

This requests that all sipXconfig profiles be automatically re-generated as 
part of an upgrade.

This the general solution for the case when the upgraded code changes what is 
generated into a given profile.  For devices that had a pre-upgrade profile, 
new profiles must be generated.  This should be done automatically by the 
upgrade, and not require any manual steps.

Two specific cases that this addresses for 4.0 are:
   - the new BLF URI format; and
   - MOH added to the Polycom 3.1 firmware.
